Patient Consent

For your convenience, we’ve made our consent forms and questionnaires available online. These documents give you a clear picture of our treatment plans. Before your visit, taking a moment to go through them can help you feel more prepared and give you the chance to note down any questions you might have.

When you visit, we’ll ask you to fill out a few standard forms — the General Patient Information & Medical Questionnaire, the Protection of Personal Information Act (POPIA) Consent Form, and the Financial Policy Consent Form. Depending on your treatment plan, there might be a few additional forms, but don’t worry. We will guide you through each one to ensure you’re confident in the care you’re receiving.
